The Vax Pack Hero free cards program and contest is offered by the Vaccine Education Center at The Children’s Hospital of Philadelphia (Sponsor). The following rules apply:
- You must be 18 years old to claim the free prize. (If you are younger than 18 years old, please have a parent or guardian review this information and complete the form.)
- One (1) free starter pack of cards per household.
- Offer valid for U.S. residents only.
- Cards will only be mailed to valid U.S. addresses.
- Full name for those 18 years and older (and the name of the parent or guardian, for those younger than 18) and valid claim code must be submitted for cards to be sent.
- Returned cards will not be sent to a new or revised address.
- This program and the prize may be discontinued or changed at any time without notice.
- The contest is subject to all applicable federal, state, and local laws and regulations and is void where prohibited.
- By claiming the Vax Pack Hero cards, you agree to release and hold harmless Sponsor, and its subsidiaries, affiliates and each such company’s officers, directors, employees and agents from and against any claim or cause of action arising out of participation or receipt or use or misuse of the Vax Pack Hero cards.
